Item is an opaque white, glass bottle with little red pills inside (10 total).
The front of the bottle reads "Anxiety Pills" in red paint. The back reads "Just as the name suggests" in slightly smaller, black painted letters.
Taking a pill will not suppress anxiety. In fact, those that consume a pill will roll 1d6 and wait that many turns to feel the pill kick in.
Once a pill is active, the effected character will suffer from deep bouts of anxiety and an overzealous concern for their safety (examples such as steering clear of open doors "because the wind could slam it shut on them and break their neck" are not uncommon), as well as disadvantage on any rolls made for the next 2D6 hours per pill consumed (maximum of 10 pills for 20D6, or 120 hours/5 days). The effected character can still force themselves to act through the fear by making a wisdom saving throw (DC 12), also with disadvantage. Making the save allows for the action to be performed, begrudgingly, but it does not remove the effects of the pill; further actions made while the pill is active require saves each time.
The pills can be broken down into a less noticeable powder and mixed into food or drink as needed.
