Wondrous Item, unknown rarity (requires attunement)

Crafted from 1 Doppelganger Skull, modified to hold two healing potions allowing the wearer to drink a healing potion without needing to have hands free.  If the wearer wildshapes, the helmet shifts with them to fit the creature the wearer has turned into.

Once per day, you can use this helmet to read the thoughts of nearby creatures. As an action, you can read the surface thoughts of one creature within 60 feet of you. The effect can penetrate barriers, but 3 feet of wood or dirt, 2 feet of stone, 2 inches of metal, or a thin sheet of lead blocks it. This effect lasts for ten minutes. While you are reading a creature’s mind, you have advantage on Wisdom (Insight) and Charisma (Deception, Intimidation, and Persuasion) checks against it.

As a bonus action, you can drink one of the health potions held in the hat even if in wildshape form as a result of the squirrel intestine tubes serving as straws.  

If the wearer rolls a Nat 1 on an attack or ability check, there is a 25% chance that one of the health potions could break.  Upon rolling a Nat 1, roll a d4.  On a result of a 1 on the d4, one of the health potions breaks and is lost.  The fitment on the helmet remains intact allowing the healing potion to be replaced in future.
