When rolling damage with this arrow, any roll less than the median roll will be counted as a median for this attack. For example a longbow would deal a minimum of 5 + your Dexterity Modifier points of piercing damage with the average roll of 1d8 being 4.5 which gets rounded up to 5.
It’s up to you to decide whether a character has proficiency with a firearm. Characters in most D&D worlds wouldn’t have such proficiency. During their downtime, characters can use the training rules in the Player’s Handbook to acquire proficiency, assuming that they have enough ammunition to keep the weapons working while mastering their use.
