An enhanced version of the normal Warforged voicebox, the acutolarynx has been adjusted to allow the Warforged to speak at pitches higher than the humanoid range of hearing. As a free action, a Warforged with the acutolarynx installed can change the pitch of their voice to a level that makes most humanoids (including the Warforged themself) unable to hear what they're saying, even if incanting the verbal component of spells.
Bestial or animalistic species (like Tabaxi and Kenku) will still hear when the Warforged speaks at this higher pitch, though they may have trouble discerning what was said as it will mostly sound like a shrill whine. Animals will react negatively to the Warforged's voice when in this state, and may either flee or attack.

Posted May 24, 2021I like the idea, but I think it could use a more practical and rules-based application. The wording is vague as it stands. Maybe it could be used to charm an animal or cause fear if the target fails a CON or WIS check contested by the warforged's Animal Handling or Performance. It would also be cool if it could be used as a musical instrument and flavor it to sound like throat singing.

Posted May 25, 2021Those features would be cool! I was trying to keep its abilities to a minimum, as it's designed to be a mid-level campaign reward, but I can definitely see the musical aspect being optionally included to bump the item from Uncommon to Rare.
You're right that I definitely could have gone into more detail regarding the effect on animals. The item was inspired by the real-life dog dazer, which makes an unpleasent high-pitched buzz inaudible to humans, so my intent was that the acutolarynx simply has a similar effect in-game.
