Ring, artifact
This ring when worn blocks the creature from divination sight and other things that would reveal it true form. Looking at a creature with true sight or something similar who is wearing this ring show a barrier. Divination spells to track this creature fail while they are wearing this ring.
The only way to overcome any of these effects is through the use of wish and it can only reveal one thing about the creature once every 24 hours.
