Potion, rare

A Box of dubious origin containing 6 untested alchemical potions. if one attempts to identify the potions in the box they take 1d4 psychic damage and are considered insane for 1 minute after which they have no recollection of the potions properties. If a creature decides to try one of the potions roll 1D6 to determine which of the following potions they get. No potion can be used more than once, if all the potions are used the box disappears in a puff of smoke and a voice says "thank you for testing Alburan's Alchemical Oddities, for all your Alchemical Needs" and a piece of paper appears with either complete gibberish or directions to Alburan's mage tower

1. potion of the paramour - Created by the mad mage Alburan to "liven up" what he considered the rather dull get-togethers of the Mages Consortium. When a creature drinks this portion, roll a 1D4 to determine how many hours the potion lasts. For 1D4 hours the creature that drank this potion becomes charmed by any and all creatures it sees regardless of species or gender, and regards all of them as objects of of its undying affection. while it is charmed, if it makes physical contact with any creature while under the effect of the potion that creature must make a DC 15 wisdom saving throw or they too will fall into the Charmed condition until the effects of the potion end. 

2. Potion of almost perfect polymorph - Created by the mad mage Alburan due to frustration brought about by his inability to speak while masquerading in his favorite polymorphed form of a yellow spotted ferret. When a creature drinks this potion roll a 1D4 to determine which of the effects the potion has, and roll a 1D6 to determine how long the effect lasts for. If the creature is reduced to 0 hit points it will remain in whatever form it was polymorphed into until the duration of the spell ends. Once the spell ends the creature will return to the hit points it had prior to the polymorph minus any hit points that cary over from when its hit points were reduced to 0 as is standard with a polymorph spell. If a creature dies while under the effects of this potion its body remains in the form it has assumed, if the creature is resurrected the polymorphed form becomes permanent (though it can be undone by a wish spell or a magic user higher than 15th level)

Effect 1: The creature is are polymorphed into another random creature (DM's choice), and the creature takes on all the stats of its new form. the creature is subject to any detriments that this form entails, however the creature cannot use any of the polymorphed form's abilities, and instead retains any class abilities it had, which can be utilized in it's new form

Effect 2: (The DM choses a random creature) The Polymorph fails but the creature that drank the potion gains whatever abilities it would have gained from its poymorphed form, including movement types, attacks, speed, and the creatures size (if it is necessary to make use of its abilities). if the creature ability requires a certain body part to be used, the creature mutates to have that body part, and is considered an Aberration.

Effect 3: The creature is Polymorphed into another creature of its choice, and takes on all the stats and abilities of its new form as in a standard polymorph spell, however it retains the ability to speak any languages it knew in its original form.

Effect 4: The creature is Polymorphed into another creature of its choice and the creatures stats are added to its own, in this for it can use both the abilities of the creature it polymorphed into, as well as any class abilities it possesses, however it cannot level up in this form, and is treated as a dangerous aberration, giving it disadvantage on all persuasion, and deception checks.

Duration: (1 - six seconds) (2 - one minute) (3 - one hour) (4 - one day) (5 - one month) (6 - permanent, though it can be undone by a wish spell or a magic user higher than 15th level)

3. potion of miscellaneous memories  - Created by the mad mage Alburan so he could sneak into and then out of the Mage's Consortium once he found it unbearably boring. When a creature drinks this potion any creature that can see them is randomly affected by the effects of an alter memory spell, that creatures memory is altered so that it believes specific things about the creature that drank the potion and any discrepancies in its memory are rectified in order to justify this belief, however only past memories are affected, and any actions the creature that drank the potion makes going forward may change how it is viewed. Roll 1D6 for each creature that see's the creature drink the potion, in order to determine the potion's effect. (the effects of this potion are permanent)

Effect 1: Hostility - The creature's memories are altered so that it believes that the creature who drank the potion is it's mortal enemy.

Effect 2: obsession - The creature's memories are altered so that it considers the creature that drank the potion to be the object of it's unhealthy obsession (the nature of this obsession is up to the DM)

Effect 3: suspicion - The creature's memories are altered so that it believes the creature that drank the potion is a shifty and untrustworthy person that owes it money (disadvantage on all persuasion rolls, and the creature may become hostile if you don't pay it)

Effect 4: admiration - The creature's memories are altered so that it believes the creature that drank the potion is an object of their admiration, this effect can be exemplified by children wanting to learn from them, store owners giving them a discount, or just a general attitude of respect (advantage on persuasion rolls)

Effect 5: friendship - The creature's memories are altered so that it believes the creature that drank the potion is a great friend and ally (the nature of this friendship is up to the DM)

Effect 6: Worship - The creature's memories are altered so that it believes the creature that drank the potion is their god and will willingly obey their comands.

4. potion of soul swapping - Created by the Mad mage Alburan when the head of the Mage's Consortium told him he wouldn't last a day in his shoes, this potion causes the creature who drinks it to swap bodies with the first creature they touch for (1D4 X 6 hours). They gain access to all of that creatures skills and abilities so long as they inhabit their body and vice versa.

5. Potion of immovable might - Created by the Mad mage Alburan to prevent people from throwing him out of their establishments, when a creature drinks this potion their body gains the effects of an immovable rod, Their movement speed is reduced to 0 however they gain the ability to lift 8,000 pounds and their strength is set at 50 for the duration, if someone attempts to move them they must make a DC 30 strength check to move them 10 feet. This potion lasts for (1D4 hours)

6. Potion of the Last hurrah - Created by the Mad mage Alburan in an attempt to take out the numerous members of the Mages consortium that were hunting him. This potion converts the drinkers remaining hit points into damage, causing every creature within a 90 foot sphere to take force damage equal to the number of remaining hit points of the creature that drank the potion, this renders the creature that took the potion unconscious.
