Wondrous Item, very rare (requires attunement by a person with at least 12 Constitution.)

The Amulet of Lost Souls is a rather expensive looking Amulet with a golden trim and a blood diamond sitting in the middle. 

While most are uncertain as to what it actually does, many a Mage can attest that it is the most handy of items as it acts like a Lich's Phylactery without the need for becoming Undead, a Cleric, or Evil.

This Amulet will let any who Attunes to it resurrect within 1d4+2 days, assuming that the amulet was undestroyed in their death such as they were disintegrated or swallowed whole, regaining 1/4 their HP after resurrection. However the downside to this Amulet  is that after successful resurrection the wearer then also gains Negative Levels equal to half their Hit Dice that can only be negated with rest or the Wish Spell.

Notes: They are also only able to resurrect in their own body as opposed to any other corpse nearby, if they've been decapitated, lost a limb, or any other such thing the amulet will regrow the severed body part. The wearer must have at least 12 Con, base, before being able to wear the Amulet. This cannot be negated with Magical Items and/or Spells. However, after their resurrection the Negative Levels will not affect the Wearers ability to Wear the Amulet, meaning that they will still be able to wear it regardless. , person with at least 12 Constitution.
