A simple wooden box with a bear print carved onto the top. Inside the box is 1d20 +5 animal shaped cookies. Which cookies from the list below are in the box is determined by the DM ahead of time.
As a bonus action, a character can grab a random cookie from the box and eat it. Alternatively, they can use an action to grab 3 random cookies, eat one, and return the other two to the box. Any animal cookies that aren't consumed or put back in this way crumble into crumbs and lose any magical effects.
When consumed, the character transforms into the animal that corresponds to the shape of that animal cookie. The transformation lasts for ten minutes, or until the target drops to 0 hit points or dies. The target's attributes, aside from Intelligence, are replaced by the attributes of the chosen beast. It retains its alignment, Intelligence and personality. The target assumes the hit points of its new form. When it reverts to its normal form, the creature returns to the number of hit points it had before it transformed. If it reverts as a result of dropping to 0 hit points, any excess damage carries over to its normal form. As long as the excess damage doesn't reduce the creature's normal form to 0 hit points, it isn't knocked unconscious. The creature is limited in the actions it can perform by the nature of its new form, and it can't speak, or cast spells. The target's gear melds into the new form. The creature can't activate, use, wield, or otherwise benefit from any of its equipment.
