Outside of a host this resembles a glowing plant root, but when a potential host touches it, it will embed itself underneath their skin and attach itself to the central nervous system invoking involuntary attunement on a successful Death Saving Throw or death. The host is noticeably has glowing veins just underneath their skin.
It understands Deep Speech but can speak telepathically with its host once bonded.
Death Aura. Once attuned, the host is able to emit a radiant damage aura of a 15ft radius for 10rds. Any other creatures starting their turn within the radiant aura takes an immediate 1d8 radiant damage. This effect is usable 3 times per long rest.
Final Sacrifice. If the host body is reduced to 0hp it begins to glow brightly. On a failed Death Saving Throw, it will explode for a pulse of radiant damage equal to the character's level x d8 in a 30ft radius. A Constitution Saving Throw may reduce damage by half. Any character that can move beyond the radius may alternatively roll a Dexterity Saving Throw for half damage.
Symbiotic Nature - This cannot be removed while you are attuned to it, and you cannot voluntarily end your attunement to it. If you are targeted by a spell that ends a curse, your attunement to it ends, and it leaves your body.
Notes: Bonus: Charisma Score, Death Aura, Death Saving Throw, Damage
