Death grew tired of other higher powers playing games with mortal lives, so they dropped four of their dice onto the material plane. That way death could ask mortals if they wanted one last roll of the dice.
The dice can appear as normal playing dice or as trinkets. They do not register as magical by detect magic or other forms of divination; they do not require attunement. The dice rarely appear together and are often a singular die or part of a mundane set.
When a creature fails its last death save, or dies instantly, the die activates, and the creature is transported to a demiplane where they are greeted by death.
Death offers to roll one final die for the creature. Explaining the below outcomes depending on the die. If the creature agrees death rolls the die and the results are binding.
Outcomes for a d4 with a (d8) and optional [d12].
1 (1-2) [1-3] The soul cannot be returned to the material plane with less than a raise dead spell. The soul will wait on the demiplane with death.
2 (3-4) [4-6] Nothing happens. the soul may be raised or choose to move on.
3 (5-6) [7-9] The soul is transported immediately to the plane of whatever entity holds greatest claim on the soul, and waits to be revived, or move on.
4 (7-8) [10-12] The creature stands up with full HP and full spell slots with the effects of a long rest, but now owes death a favor.
Outcomes for a d6 optional [d12]
1 [1-2] The soul cannot be returned to the material plane with less than a raise dead spell. The soul will wait on the demiplane with death.
2 [3-4] When revived the soul will return as a hollow one or reborn
3 [5-6] Nothing happens. the soul may be raised or choose to move on.
4 [7-8] The soul is transported immediately to the plane of whatever entity holds greatest claim on the soul, and waits to be revived, or move on.
5 [9-10] The creature awakens with 1d6 [1d12] Hp and owes death a favor.
6 [11-12] The creature stands up with full HP and full spell slots with the effects of a long rest, but now owes death a favor.
Regardless of the result of the roll the creature retains full memory of their time while dead and awakens holding the dice in their hand regardless of where they stored it.
