This figurine acts as a focus for a druid's wild shape ability. It can come in many forms, a whitled wooden figurine, chisled stone, a knitted doll, etc. It is created specifically for an individual and is used by them to transform into the animal it is made to look like. In the process of the transformation the item itself first transforms into a full sized version of its appropriate animal and then the individuals consciousness transfers into it. The now lifeless human body then transforms into a figurine of similar appearance to them for the duration of the wild shape.
This transformation can only take place if the Nature Totem is within 30ft of the individual. This restriction applies both ways meaning they cannot willingly turn back to their human form if the doll is further than 30ft from them. If the individual dies as a beast it takes 1d4 hours for the soul to find and return to the humanoid totem if it is further than 60ft away.
Nature Totems can only be created for beasts the individual has encountered before as with any wild shape magic. Creating a detailed enough totem can be done by any Circle of the Totem Druid in 10 minutes and 1 gold piece per challenge rating of the beast (beasts with CR < 1 take the same as a CR1 beast). Imbuing the item with magic required to make it a Nature Token however takes 50 minutes and 49 gold pieces per challenge rating of the beast (beasts with CR < 1 take the same as a CR1 beast) by someone proficient in Animal Handling.
Other than the above rules the Wild Shape feature uses the same rules and restrictions as dictated by the Druid class and Subclass, eg casting time.
The Nature Totem can be thrown as a bonus action up to 30ft. Roll a Dex Check, the "AC" of the square you're aiming for is 5-10 (DM's discretion based on the difficulty of the throw). If you miss the toy lands 5ft away in any direction for each point under the AC (DM's discretion).
