This small, leather-bound old pocketbook served as a journal for notes of an adventurer once upon a time. The pages are filled with shorthand and quick sketches of encounters the adventurer experienced. There are additional pages inserted here and there and items are taped onto some pages. While it does seem rather helpful, it isn't always the most accurate, since the adventurer wasn't an expert in every creature they came across. The notes are sometimes difficult to decipher. However, there are notes on what the adventurer learned about the creatures, and occasionally on different locations they explored, and knowledge is certainly contained within these pages.

When you encounter a creature, you can use an action to make a DC 12 Wisdom (Animal Handling) check. On a success, if it is a creature the adventurer encountered, you find the page it's been written on. It has the name, a picture, and a short informational paragraph about it. The entry is only on the species of the creature and does not mention any specifics on the individual. To determine if the creature has been noted in the journal and what information the previous adventurer was able to compile and whether they were accurate, The DM will check the creature's CR 

If the creature's CR is 1/4 your lv or below you automatically succeed 
If the creature's CR is 1/2 your lv roll a d20 with advantage, as the DM rolls a hidden one. If the total is above 10 + the creatures wisdom, you succeed 
If the creature is your lv the DM, roll the same and if the total is 15 + the creatures wisdom, you succeed 
If the creature is 1 lv above yours roll with disadvantage. If the total is 15 + the creatures wisdom, you succeed 
Any lv higher auto fails 
The DM never reveals if it succeeds or fails, they simply reveal what the page you found says. On a successful search, the entry will have mostly accurate information including a quick description, fun fact they may have learned, something they found they're resistant to or vulnerable to, an alignment or allies they've noticed, and/or an ability it may have.
If the player fails the adventurer may not have been entirely accurate, and the DM reveals false information about the creature or the creature may not be found at all.
When you encounter specific terrain, territory, or path, you may make a DC 15 Wisdom (survival) check. On success, you're able to open the notebook up to an entry on it, if the adventurer previously encountered it. At the discretion of the DM, some paths or areas likely wouldn't have been accessible to a previous adventurer, and may not be found, however on a success you would know for certain it simply wouldn't have been accessible to the author. Entries are quick descriptions, roll a d20 with the DM rolling a hidden one. If the total is above 15, you find accurate info which may include spots to watch out for, rumors, often encountered creatures, or observations. On a fail, the entry is either not very thorough or may have false or inaccurate information.
