When you drink this potion, you fall unconscious for 1 minute and become cursed with lycanthropy, following the rules described on page 207 of the Monster Manual and page 155 of The Impermissicon. The type of lycanthrope that you become depends on the type of the elixir that you drank.
The DM chooses the type of elixir or determines it randomly from the options below.
