Weapon (dart), very rare

This arrow is made of warped flesh and bone. It has a blinking eye on the tip. It deals an extra 1d6 psychic damage on a hit. It fuses to the target creature’s body on a hit, giving them disadvantage on cha checks and con saves until it is surgically removed. Roll a d4, on a 1, it fuses to an arm or similar appendage, giving the target disadvantage on attack rolls and ability checks that involve the affected appendage. On a 2, it fuses to a leg or similar appendage, giving them disadvantage on dex saves and acrobatics checks as well as reducing their speed by 5 ft. On a 3, it fuses to the target's abdomen, giving them disadvantage on str checks and saves. On a 4, it fuses to the target's head, giving them disadvantage on int and wis checks and saves. Regardless of what you rolled on the d4, the arrow gives the target disadvantage on cha checks and con saves in addition to the penalties from the d4 roll until it is surgically removed.
