Wondrous Item, very rare (requires attunement by a Wearing the amulet for a short rest, or for 4 hours without rest.)

This amulet is carved from an unfamiliar black metal and hangs on a thin chain of gold.

The amulet glows a deep red when the wearer is within 120 feet of a demonic fiend. The first time it does so a simple carved image of a gargoyle appears and remains as long as the wearer remains attuned.

The wearer receives +2 to Deception as well as advantage on any saving throws to avoid Madness.

 Cursed - This amulet is cursed, and becoming attuned to it extends the curse to you. As long as you remain cursed, you are unwilling to part with it or remove it. This amulet was crafted by followers of Fraz-Urb’luu, Prince of Deception and Demon Lord of Illusions, with his blessing. Whenever it begins to glow red due to the presence of a demon, Make a DC 20 Wisdom saving throw. On a failure the DM will select a target at random from all creatures visible to the wearer.  This creature shall appear to the wearer as whatever fiend caused the amulet to activate. The wearer will be filled with a powerful feeling of righteous duty to smite this fiend. On any turn the wearer does not attack or otherwise attempt to harm this creature they must make a DC 20 saving throw, Wisdom or Charisma (wearer's choice) or receive a level of Madness. (Out of the Abyss - modify for other settings)

Any efforts to reveal or dispel the illusion will be understood by the wearer as demonic trickery and ignored. The effect ends when the creature drops to 0 hit points, there are no actual demonic fiends within 120 feet, or when the curse is removed.
