This exquisite amulet, bedecked with the timeless symbol of reincarnation, embodies the essence of renewal and the mysterious force that governs both life and death.
Rebirth. The first time you die while wearing this amulet and it remains on your corpse for 1 hour, you will rise again, having your hit points restored to their maximum. When you are resurrected in this way, you receive the Curse of Undeath trait.
Curse of Undeath. After being resurrected by the amulet for the first time, you cease aging and gain the Undead creature type. Any effect that applies to Undead creatures now applies to you. If the amulet is destroyed, you stop wearing it, or you become unattuned to it, you die instantly, and your body is reduced to ash.
Phylactery. While under the effects of the Curse of Undeath, the Amulet of Rebirth acts as your phylactery. If you die while wearing the amulet, your corpse will turn to ash, but none of your equipment will be destroyed. 24 hours later, you will be reincarnated, as if by the spell 'Reincarnate'. Your new body will form in the space of the amulet, with it draped around your neck. If there is no room for your new body to be reincarnated in the space of the amulet, you do not reincarnate until ample space is made available.
