Item properties: this item can be used as a spellcasting focus. It is a tiny wicked doll on a string, and it looks like it should be worn as an necklace. When not being observed directly, it moves in a way which imitates breathing.

Attunement ritual: when trying to attune to this item, you will take [(1d6 + 4) * your level] necrotic damage over course of 1 minute. If you retain consciousness over the course of this effect, then you will attune to this item. If you fail, then you will not be able to try to attune to this item for next 7 days. You cannot cast spells or use class/race features while attuning to this item. If you get reduced to 0 hit points while trying to attune to this item, then you instantly die and your corpse gets disintegrated.

Living Doll (while attuned): your creature type becomes Construct. You no longer need to eat, drink or sleep, and you benefits from a long rest by remaining in a motionless state for 4 hours. You also gain one special modification of your choice, and you can switch the chosen effect for a new one at the end of a long rest.
Macabre Dancer Robes - an undispellable illusionary effect changes appearance your armor/cloths you wear to appear as gothic dancing dress or suit. You can now take Disengage action as a bonus action. Moreover, in combat you can use your reaction to inflict 2 necrotic damage to yourself to gain 7 temporary hit points.
Evil Plush Paws - an undispellable illusionary effect changes appearance of your arms to look plush paws. You can now perform Grapple action as a bonus action. Moreover, your unarmed strikes now deal (1d12 + your Strength modifier) bludgeoning damage.
Toy Soldier Uniform - an undispellable illusionary effect changes appearance your armor/cloths you wear to appear as a military uniform. You can now take Hide action as a bonus action. Moreover, as reaction you can expend up to 5 of your hit points to gain bonus AC until start of your turn equal to the amount of your hit points spent.
