The crystalized brain of a mind flayer. Created through immense heat, pressure, and psychic distress, the object contains within it an impression of the illithid’s final fleeting thoughts.
You can use the cerebrum as a spellcasting focus while you hold it. When you use a Metamagic option on a spell while you are holding the brain, immediately after casting the spell you can teleport to an unoccupied space you can see within 30 feet of you.
Curse of Megalophobia. While attuned to this item, whenever you are damaged by a creature at least two sizes larger than you, you must succeed on a Wisdom saving throw or become frightened of that creature for 1 minute. The DC equals 10 or half the damage you take, whichever number is higher.
While you are frightened in this way, you must spend your turns trying to move as far away from the creature as you can, and you can't willingly move to a space within 30 feet of the creature. you also can't take reactions. For your action, you can use only the Dash action or try to escape from an effect that prevents you from moving. If there’s nowhere you can move, you can use the Dodge action. At the end of each of your turns, you can repeat the saving throw, ending the effect on yourself on a success.
