This ancient amulet, crafted from silver and set with a crescent moonstone, glows faintly under the light of the moon. Intricate engravings of undead creatures and moon phases cover its surface, hinting at its eerie power.

Transformation Ability: While you are attuned to the Amulet of the Dead and standing in moonlight, you can use an action to transform into either a Zombie or a Skeleton. You can remain in this form for up to 1 hour, or until you use an action to revert to your original form, are reduced to 0 hit points, or no longer stand in moonlight. Once you revert to your original form, you can’t transform again until the next dawn.

• Zombie Form: Your game statistics are replaced by the statistics of a Zombie, but you retain your alignment, personality, and Intelligence, Wisdom, and Charisma scores. You also retain all of your skill and saving throw proficiencies, in addition to gaining those of the creature. If the creature has the same proficiency as you and the bonus in its stat block is higher than yours, use the creature’s bonus instead of yours.
• Skeleton Form: Your game statistics are replaced by the statistics of a Skeleton, under the same conditions and rules as the Zombie form.

Curse: The amulet is cursed, a fact that is revealed only when an identify spell is cast on it or you attune to it. Attuning to the amulet curses you until you are targeted by the remove curse spell or similar magic. Removing the medallion fails to end the curse on you. While cursed, you cannot regain hit points through magical means, and if you die while wearing the medallion, you automatically transform into a Zombie under the DM’s control, unless you are resurrected by a spell like true resurrection or wish.

Moonlit Resilience: While you are in your undead form and under moonlight, you gain resistance to bludgeoning, piercing, and slashing damage from nonmagical attacks.

Drawbacks: Each time you transform back into your original form, you must succeed on a DC 15 Constitution saving throw or gain one level of exhaustion, as the transformation takes a toll on your living body.
