Wondrous Item, legendary (requires attunement by a creature that has previously assumed a different form)

This oddly thin tome is bound with a network of animal hides, and has a segmented spine that slowly undulates. There is only one copy of this book, but all record of its existence vanish as quickly as those who dare to use it. Few ancient scholars, who often hide their own knowledge of it, believe the book itself is a god of transformation somehow cursed to take its sole form. When you first interact with the book, you will find two pages: one describing you in great physical detail, and a second page with unreadable symbols whose meaning cannot be gleaned by magical means.

If you can attune to this item, the second page becomes readable and explains the book's function. In addition, new pages will immediately burst from the spine and fill out the book, each one describing every creature's species you have come across.

While attuned to this book, once per long rest, you can use an action to cast the spell shapechange without requiring any components or spell slots. When you use the book to do so, you can only assume a form with a Challenge Rating no higher than 1/4 of your level or Challenge Rating.

Curse. This tome is cursed. Attuning to it spreads the curse to you. While cursed, each time you transform using any type of magic, the DM rolls 1d100. On a 2 or lower, your new form becomes permanent and you lose the ability to transform yourself in any way. The chance of this happening increases by 2 with each transformation. When this happens, the book devours all pages and magically vanishes.

In addition, each time you revert back to your normal form, you retain a minor trait from your transformation at the DMs discretion.
