To use Ao's Left Blue Eye, the PC must accept it as a permanent replacement of his or her eye.
Ao's Left Blue Eye provides +4 to Arcana, Spell Attacks, Spell Damage & Spell's Save DC. And it adds the Disintegrate spell (can cast once per day).
