The Hunger of the Maw

Your fanged bite is a natural weapon, which counts as a magical melee weapon with which you are proficient. You add your  Strength modifier, to the attack and damage rolls when you attack with this bite. It deals 1d6 slashing damage on a hit. 

  • When you attack with this bite and hit a creature that isn’t a Construct or an Undead, you can empower yourself in one of the following ways of your choice:
    • You regain hit points equal to the slashing damage dealt by the bite plus your proficiency bonus.
    • You gain a bonus to the next ability check or attack roll you make; the bonus equals the slashing damage dealt by the bite.
  • You can empower yourself with this bite a number of times equal to your proficiency bonus, and you regain all expended uses when you finish a long rest.

Curse. This item is cursed, and becoming attuned to it extends the curse to you. As long as you remain cursed, you are unwilling to part with the item, keeping it on your body at all times. 

Whenever your health is under 20% you must succeed on a DC 15 Wisdom saving throw or go into a feeding frenzy. This state ends when you start your turn and there are no creatures within 60 feet of you that you can see or hear or when your health is over 50%.

While in a feeding frenzy, you regard the creature nearest to you that you can see or hear as a food source. If there are multiple possible creatures, choose one at random. On each of your turns, you must move as close to the creature as possible and take the bite Attack action, targeting the creature. If you’re unable to get close enough to the creature to attack it with your bite, your turn ends after you’ve used up all your available movement. If the creature dies or can no longer be seen or heard by you, the next nearest creature that you can see or hear becomes your new target.
