Traits

Undead Fortitude. If the creature would be reduced to 0 hit points, there is a chance that it can instead only be reduced to 1. This feature is negated if the type of damage received is radiant.

Actions

Multiattack. The undead dragon turtle makes three attacks: one with its bite and two with its claws.

Bite. Melee Weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 15 (2d10 + 4) piercing damage plus 7 (2d6) necrotic damage.

Claw. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 11 (2d6 + 4) slashing damage.

Poison Breath (Recharge 5–6). The undead dragon turtle exhales a spray of vile toxin in a 40-foot cone. Each creature in that area must make a DC 14 Constitution saving throw, taking 42 (12d6) poison damage on a failed save, or half as much damage on a successful one.

Roar. The undead dragon turtle roars loudly. All creatures within 10 feet must make a DC 14 Dexterity saving throw. On a failed save, the creature takes 18 (4d8) force damage and is knocked prone. On a successful save, a creature takes half as much damage and isn’t knocked prone. Unsecured boats within 10 feet are pushed back 5 feet.

Description

Once a placid dweller of the deep, this dragon turtle has been corrupted by a curse that drove it mad and rendered it unable to die. It will aggressively attack anything that wakes it, as well as anything nearby.
