Charge. If the centaur moves at least 30 feet straight toward a target and then hits it with a pike attack on the same turn, the target takes an extra 10 (3d6) piercing damage. This can only occur once per turn.

Multiattack. The centaur makes five attacks: One with its hooves, two with its pikes and two with its longswords. Any of these attacks can be substituted with a longbow attack.

Legendary Resistance(1/Day). If the centaur fails a saving throw, it can choose to succeed instead.

Longsword. Melee Weapon Attack: +10 to hit, reach 5 ft., one target. Hit: 9 (1d8 + 5) slashing damage.

Pike. Melee Weapon Attack: +10 to hit, reach 10 ft., one target. Hit: 10 (1d10 + 5) piercing damage.

Hooves. Melee Weapon Attack: +10 to hit, reach 5 ft., one target. Hit: 15 (3d6 + 5) bludgeoning damage.

Longbow. Ranged Weapon Attack: +7 to hit, range 150/600 ft., one target. Hit: 11 (2d8 + 2) piercing damage. 

Legendary Actions

The centaur can take 2 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. The centaur regains spent legendary actions at the start of their turn.

Attack. The centaur makes any one of its attacks.

Frightening Gaze(Costs 2 actions). The centaur fixes its gaze on one creature it can see within 30 feet of it. The target must succeed on a DC 18 Wisdom saving throw or become frightened for 1 minute. The frightened target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. If a target's saving throw is successful or the effect ends for it, the target is immune to the centaur's gaze for the next 24 hours.

Description

It is clear that this creature was once a centaur; now, it is something far greater but twisted. Its already impressive physique is even bulkier. Its human torso is elongated, the creature's full height being 15 ft. Occupying this extra space are not two, but six massive arms. The centaur has long, bedraggled hair that flows as the centaur gallops through the wilderness. From that high up, the creature's facial features are hard to discern, but its cruel gaze can be seen -and felt- from far away.

Somehow, the centaur has managed to equip itself for battle. Plate mail covers its huge chest. Its top pair of arms hold a sizable longbow. Its middle pair of arms hold a pike each, and its bottom pair hold a longsword each.

All of these physical advantages allow the Abominable Centaur to tap into its primal killing instincts. It roves the wilderness, hunting any living creature in its path to sustain its voracious appetite. Occasionally, it raids towns and villages in search of better equipment.
