Bite. Melee Weapon Attack: +1 to hit, reach 5 ft., one target. Hit: 2 (2d6 +1) piercing damage.

Eye Ray. The Xao Eth shoots a magical eye ray from it central eye. Roll randomly only for the first attack and then that is the same attack it uses thereafter. The number of targets depends on the type of ray.

  1. Orange Eye/Ray. The Xao Eth shoots an orange ray in a line up to 50'. Any creature in the line must succeed on a DC 13 Dexterity saving throw or suffer 2d10 Fire Damage.
  2. Green Eye/Ray. A single target the Xao Eth can see must succeed on a DC 13 Constitution saving throw or be paralyzed for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
  3. Red Eye/Ray. A single target that the Xao Eth can see must succeed on a DC 13 Constitution saving throw or suffer 2d8 Force damage and the Xao Eth heals for the same amount.
  4. Purple Eye/Ray. A ray spreads in a 30' range 15' cone. Any creature within the cone must succeed on a DC 13 Wisdom saving throw or be driven mad for 1 minute. During this madness, the target(s) will attack random creatures being allies or enemies within 5' of them using their current melee attack. The target(s) can attempt another saving throw at the end of each of their turns. If a target is successful on the saving throw, they immediately suffer 2d4 Psychic damage and are immune to this eye ray for a full minute. 

Teleport (Recharge 5-6). The Xao Eth can teleport up to 90' to an empty space.

Reactions

Spell Absorption. If the Xao Eth is targeted by a spell and makes its Saving Throw, the Xao Eth does not suffer any effect from the spell and, instead heals 2d6 Hit Points. It sacrifices the use of its eye power on its next Attack to do this and can only attack with its teeth instead. 

Description

A Xao Eth is aptly named a Chaos Eye. Though the creatures look largely similar to one another in appearance, when encountering them their one central eye color determines a chaotic power that will imamate from it.  

Xao Eth are found in deep mountain caverns, deserted swamplands, in the Underdark, and in some places of the Abyss.
