Paralysing Gaze. When a creature that can see the Dread Gorgon's eyes starts its turn within 30 feet of the Dread Gorgon, the Dread Gorgon can force it to make a DC 14 Constitution saving throw if the Dread Gorgon isn't incapacitated and can see the creature. If the saving throw fails by 5 or more, the creature is instantly paralyzed. Otherwise, a creature that fails the save begins to become paralyzed and is incapacitated, and has its speed reduced by half. The incapacitated creature must repeat the saving throw at the end of its next turn, becoming paralyzed on a failure or ending the effect on a success. The paralyzation lasts until the creature is freed by the greater restoration spell or other magic.

Unless surprised, a creature can avert its eyes to avoid the saving throw at the start of its turn. If the creature does so, it can't see the Dread Gorgon until the start of its next turn, when it can avert its eyes again. If the creature looks at the Dread Gorgon in the meantime, it must immediately make the save.

If the Dread Gorgon sees itself reflected on a polished surface within 30 feet of it and in an area of bright light, the Dread Gorgon is, due to its curse, affected by its own gaze. If the polished surface has been enchanted with the imprisonment spell the Dread Gorgon is sucked in to the mirror and imprisoned there until the mirror is broken, or targeted by a successful dispel magic.

Turning Defiance. The Dread Gorgon has advantage on saving throws against effects that turn undead.

Undead Fortitude. If damage reduces the Dread Gorgon to 0 hit points, it must make a Constitution saving throw with a DC of 5 + the damage taken, unless the damage is radiant or from a critical hit. On a success, the Dread Gorgon drops to 1 hit point instead.

Actions

Multiattack. The Dread Gorgon makes three melee attacks; one with its snake hair and two with its claws. The Dread Gorgon may replace one of its attacks with a bite attack.

Snake Hair. Melee Weapon Attack: +5 to hit, reach 5 ft., one creature. Hit: 4 (1d4 + 2) piercing damage plus 14 (4d6) necrotic damage.

Claws. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 10 (2d6 + 3) slashing damage. If the target is a creature other than an undead, it must succeed on a DC 14 Constitution saving throw or be incapacitated for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Bite. Melee Weapon Attack: +4 to hit, reach 5 ft., one willing creature, or a creature that is incapacitated, or paralyzed. Hit: 6 (1d6) piercing damage plus 7 (2d6) necrotic damage. The target's hit point maximum is reduced by an amount equal to the necrotic damage taken, and the Dread Gorgon regains hit points equal to that amount. The reduction lasts until the target finishes a long rest. The target dies if this effect reduces its hit point maximum to 0.

Description

While stories of the legendary Medusa frighten adventurers, it is tales of a Medusa that won't die that truly terrify! The Dread Gorgon is a Medusa that delved too deep into necromantic magics and paid the price. Scholars disagree on the exact details, but it is hypothesized that the first Dread Gorgon was a Medusa who was experimenting with various undead. Killed by her subjects, she rose from the dead with some of their abilities. Delighted by her new powers, that original Dread Gorgon must have taught what it learned to others, so that reports of more than one have occasionally been heard.
