Thrall Riders.  All attacks on a Yaggoth hit their thrall as they scuttle and dodge around the thrall's body. If a thrall is brought to 0 HP the Yaggoth is hit and killed. If a thrall is brought to 1 HP, the Yaggoth will leap off and Swarm New Thrall if one is within 20 feet or else flee.

Unnatural Vigilance. The eyes moving around the Yaggoth's skin are never still. They get +6 to their Initiative roll (+3 if asleep)

 

Actions

Thrall Weapon Attack - Scimitar/Shortsword/Axe Melee Weapon Attack: +3 to hit, reach 5 ft., one target. Hit: 4 (1d6 + 1) slashing damage.

Swarm New Thrall. A Yaggoth without a thrall will Frog-Leap onto a new thrall (opposed DEX check) if within 10 feet. Grapple attack with oversized, long-nailed hands: +3 to hit (Disadvantage vs Heavy Armor).Hit: 5 (2d4) piercing damage.

Harvest Eye. Grapple Attack: +3 to hit, range 0 ft., one target. On a natural 20 one eye is plucked out and absorbed by the Yaggoth (victim has Disadv on all rolls). If a victim is incapacitated they auto hit to take both eyes, Charming the victim who becomes their new thrall with full HP.

Hideous Giggles. Ranged Spell Attack, min range 30 feet, max range 60 ft, multiple targets. Targets must make a CHA save at DC 10 or take 1d4 psychic dmg at the start of each round until save is made. If any Yaggoth thrall takes damage the giggles from all Yaggoth stop for the rest of the encounter. Once a target makes their save they're immune for the rest of the encounter.

Frog-Leap. Yaggoth can leap 10 feet in any direction when stationary, scuttling, or from their thralls.

Description

Yaggoth are the size and shape of naked hairless children, with four arms, many-jointed hands, and unnaturally long nails. They have no legs, no sex organs, and no eyes. To make up for this they harvest eyes from other creatures. The harvested eyes are placed in cuts the Yaggoth makes in its own skin. The blinking eyes then migrate around the Yaggoth's body while the victim serves it as a Charmed thrall and steed. Thralls act as sighted when ridden, and are Incapacitated when abandoned. They are non-sentient and utter constant moans of torment. The more sets of eyes a Yaggoth has, the more powerful the creature it generally rides (1 pair of eyes per HD of thrall).

Once a Yaggoth is dead, the migrating eyes can be popped out like grapeseeds. If placed in the sockets of the thrall they were taken from, the thrall's soul is released from the dark netherworld it was consigned to. It regains its senses for a moment before dying in peace. 

If a thrall is called by name when their eyes are replaced they can make a CHA save with a DC of 10 + days of servitude. On success they are dying at 0 HP instead of dead.
