Multiattack. The Centipede makes two attacks: one with its bite and one with its tail.
Bite. Melee Weapon Attack: +4 to hit, reach 5 ft., one creature. Hit: 8 (1d6 + 2) piercing damage, and the target must succeed on a DC 11 Constitution saving throw or take 10 (3d6 +2) necrotic damage. If the necrotic damage reduces the target to 0 hit points, the target is stable but poisoned for 2 hours, even after regaining hit points, and is paralyzed while poisoned in this way. If the target is paralyzed, the Centipede can use its reaction to make a Tail attack against the target.
Tail. Melee Weapon Attack: +3 to hit, reach 5 ft., one target. Hit: 3 (1d4 + 1) bludgeoning damage plus 3 (1d6) necrotic damage.
Constrict. Melee Weapon Attack: +6 to hit, reach 5 ft., one creature. Hit: 13 (2d8 + 4) bludgeoning damage, and the target is grappled (escape DC 16). Until this grapple ends, the creature is restrained and the snake can't constrict another target.
