Actions

The stirge can detach itself by spending 5 feet of its movement. The target or a creature within 5 feet of it can detach the stirge as an action.

Posted Feb 16, 2025I think the automation on these is incorrect. It's doing necrotic with the attack and not adding an initiative effect for the sustained damage while attached

Posted Feb 18, 2025The necrotic is correct. In the new Monster Manual, necrotic represents any life-draining damage or effect, including blood loss.
Not sure what you're implying by an "initiative effect".

Posted Jul 12, 2026The way I'm reading it and most likely the way that @Bedlymn is reading it, is that it's dealing its necrotic damage on the turn that it is attacking to attach to you. So instead of just dealing 1d6 + 3 when it attacks and attaches to you, it's dealing 1d6 + 3 + 2d4 on its first turn. The necrotic damage should only come after on the Stirge's second turn but for some reason on Foundry it is happening on turn 1.
When we played on Foundry it was easily dealing well over 10+ damage on its first turn, enough to 1 shot most level 1 characters.

Posted Jul 13, 2026The rules for the attack make it pretty clear:
1. The stirge isn't attached until it makes a successful probiscis attack. Once attached, it can't make this attack any more.
2. The start of of any creature's turn occurs before the creature takes its first action. So, fundamentally, the stirge can't inflict the necrotic damage before the attack that creates the "attached" condition, and it can't inflict it after the attack because it's no longer the start of the turn.
The only loophole is whether the stirge can inflict the necrotic damage, detach, then attack again. It's not explicit, but i always figured RAI the attached drain uses up the stirge's action, so it can never inflict both. But that is no explicitly spelled out.
