Howl of Pain. The first time the Abomination of Despair takes damage it unleashes this howl of pain. Any creature within 30ft of the creature must make a DC 17 Wisdom Saving Throw or become frightened until the end of their next turn. When you succeed on this saving throw you are immune for the Howl of Pain for the next 24 hours.

Untamed Hunger. Whenever this creature uses his swallow ability it regain hit points equal to the damage dealt.

Multi-headed. Abomination of Despair has advantage on Wisdom (Perception) checks and on saving throws against being blindedcharmeddeafenedfrightenedstunned, or knocked unconscious.

 

Actions

Multiattack. The abomination of despair makes two hook arm attacks, 2 spiked hand attacks and 1 harpoon arm attack.

Hook Arm. Melee Weapon Attack: +11 to hit, reach 60 ft., 1 target. Hit: 20 (2d12 + 6) piercing damage. 

Spiked hand. Melee Weapon Attack: +11 to hit, reach 10 ft., 1 target. Hit: 30 (4d10 + 6) slashing damage, and the target is grappled (escape DC 19) if it is a Large or smaller creature. The Abomination of Despair swallows the creature if its medium or smaller. A swallowed creature is blinded and restrained, has total cover against attacks and other effects outside the Abomination of Despair. The creature takes 12 (3d6) Necrotic damage. Which the Abomination of Despair heals.

The Abomination of Despair’s stomach can hold up to two creatures at a time. If the Abomination of Despair takes 20 damage or more on a single turn from a creature inside it, the Abomination of Despair must succeed on a DC 20 Constitution saving throw at the end of that turn or regurgitate all swallowed creatures, each of which falls prone in a space within 10 feet of the Abomination of Despair. If the Abomination of Despair dies, a swallowed creature is no longer [Tooltip Not Found].

Harpoon Arm. Melee Weapon Attack: +11 to hit, range 60 ft., 1target. Hit: 26 (4d8 + 6) piercing damage, and the creature is grappled (escape DC 15) if it is a Large or smaller creature. The Harpoon Arm can only grapple one creature at a time. The Abomination of Despair can choose to let go of a grappled creature requiring no action.

Reactions

Tightened Embrace. Whenever the Abomination of Despair takes damage while it has creatures grappled, those creature take 4d10 psychic damage.

Legendary Actions

The Abomination of Despair can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The Abomination of Despair regains spent legendary actions at the start of its turn.

Description

As all beings of sorrow are painfully combined into 1 distorted and sorrowful creature the Abomination of Despair is born. Out of his body all parts of the Sorrowsworn are visible. 3 huge spiked arms, 2 hooks and a harpoon arm sticking out of his sides. 2 huge jawed heads on the place where his shoulder should be. This moan of pain coming from his entire body and multiple heads that are slightly visible everywhere under his skin.
