Homebrew [CENSORED] Species Details

Not a single living creature that faced [CENSORED] retained its sanity. Thus, every exact detail of this creature has been buried in oblivion.

The following information remains no more than a vague reconstruction based on the wounds of the few escapees who managed to flee.

Beneath the shifting red strips marked [CENSORED], the creature appears to possess the combined shape of a horse and a scorpion. When it encounters a dead creature, part of its body assumes the shape of a drill and extracts every remaining trace of essence from the corpse.

Whenever [CENSORED] reduces a creature to 0 Hit Points, it attempts to crush the body with enough force to scatter its organs throughout the surrounding area. Whether this is a deliberate attempt to prevent resurrection or an act of pure sadism remains unknown.

Any attempt to describe [CENSORED] causes severe mental corruption in both the speaker and the listener. Recording any depiction of this [CENSORED] is therefore forbidden.

Uncertain Anatomy

No surviving account agrees on the exact durability or defensive capabilities of [CENSORED].

When [CENSORED] enters combat, the DM secretly determines its Armor Class, choosing a value from 21 to 27, and its Hit Point Maximum, choosing a value from 490 to 520.

These values remain unchanged until Initiative ends.

A feature, spell, or other effect that would reveal [CENSORED]’s exact Armor Class, current Hit Points, or Hit Point Maximum instead reveals only the corresponding range.

The DM should not disclose the chosen values through narration, visible wounds, or other indirect information. Even when [CENSORED] is near death, its condition remains impossible to determine with certainty.

Combat Uses

When a feature is marked 3/Combat, [CENSORED] can use it three times during the same combat. It regains all expended uses when Initiative ends.

Legendary Resistance (3/Day)

If [CENSORED] fails a saving throw, it can choose to succeed instead.

Magic Resistance

[CENSORED] has Advantage on saving throws against spells and other magical effects.

Humongous Speed

[CENSORED] can move with horrifying speed despite its enormous body, though it rarely uses the full extent of this mobility.

Its movement does not provoke Opportunity Attacks from creatures suffering from its A Sight for Sore Eyes trait.

A Sight for Sore Eyes

At the start of its turn, a creature that can see [CENSORED] must make a DC 23 Wisdom saving throw.

The first saving throw a creature makes against this trait during a combat is made with Disadvantage. Any subsequent saving throws are made normally.

On a failed save, the creature loses control of its mind and body. It involuntarily empties itself, collapses into a frantic tantrum, falls Prone, and has the Frightened condition until the end of its next turn.

After a creature with a Wisdom modifier of +1 or higher fails this saving throw, it is immune to A Sight for Sore Eyes until Initiative ends.

A creature with a Wisdom modifier of +0 or lower gains no such immunity and must continue making the saving throw at the start of each of its turns while it can see [CENSORED].

Unless Surprised, a creature can avert its eyes at the start of its turn. If it does so, it cannot see [CENSORED] until the start of its next turn and is unaffected by this trait during that time.

If the creature looks at [CENSORED] before then, it must immediately make the saving throw.

[CENSORED] Residue

Whenever [CENSORED] moves across a surface, makes a physical attack, or physically interacts with a creature, object, or structure, it leaves detached [CENSORED] residue at every point of contact.

Each 5-foot space that [CENSORED] moves through becomes contaminated with this residue.

A creature must immediately make the saving throw described in A Sight for Sore Eyes when it does any of the following:

  • Enters a contaminated space for the first time on a turn

  • Starts its turn in a contaminated space

  • Directly touches the residue

  • Is hit by an attack that brings [CENSORED] into physical contact with it

A creature cannot avert its eyes to avoid a saving throw caused by physical contact with the residue.

Detached residue corrodes and completely degrades 1 hour after being deposited.

Essence Extraction

When [CENSORED] enters the space of a dead creature or starts its turn within 5 feet of one, it can reshape part of its body into a drill and extract every remaining trace of essence from the corpse without using an action.

The corpse is reduced to a hollow and spiritually empty husk. It cannot be restored to life by an effect that requires access to the original body.

Only an effect capable of creating a new body, such as true resurrection or wish, can restore that creature to life.

Organ Scatter

When [CENSORED] reduces a creature within its reach to 0 Hit Points, it can use its Reaction to crush the fallen creature.

The creature immediately suffers two failed Death Saving Throws.

If this kills the creature, its body is destroyed and its organs are scattered throughout a 30-foot-radius area centered on its space.

The creature can be restored to life only by an effect capable of creating a new body.

Multiattack

[CENSORED] makes three Smash attacks.

Smash

Melee Weapon Attack: +22 to hit, reach 15 ft., one target.
Hit: 38 (7d6 + 14) Bludgeoning damage.

[CENSORED] strikes with one of its savage [CENSORED] appendages, pulverizing everything caught beneath it.

Sting (3/Combat)

Countless tentacle-like [CENSORED] extremities emerge from its body and wrap around one another, forming an enormous, multiheaded [CENSORED] stinger.

Roll 1d10. [CENSORED] chooses up to that many creatures it can see within 30 feet of itself and makes a separate attack against each target. A creature can be targeted only once by the same use of Sting.

Melee Weapon Attack: +11 to hit, reach 30 ft., one target.
Hit: 39 (8d8 + 3) Piercing damage.

Engulf (3/Combat)

[CENSORED] opens its [CENSORED] upper body and attempts to cage one Large or smaller creature it can see within 10 feet of itself.

The target must make a DC 23 Dexterity saving throw.

On a successful save, the creature avoids the opening and takes no damage.

On a failed save, the creature is drawn inside [CENSORED]. Roll 1d4. The creature remains engulfed until the end of its turn that many turns later unless it escapes.

While engulfed, the creature has the Blinded and Restrained conditions, has Total Cover against effects originating outside [CENSORED], and takes 35 (14d4) Acid damage at the start of each of its turns.

[CENSORED] can contain only one creature at a time.

One Chance to Escape

At the start of one of its turns, before taking the Acid damage, the engulfed creature can immediately expend its action to make a DC 24 Strength saving throw.

On a successful save, the creature tears its way free, takes no Acid damage that turn, and appears in the nearest unoccupied space beside [CENSORED].

On a failed save, the creature remains engulfed, its action is expended, and it takes the Acid damage normally.

A creature can attempt this saving throw only once during each use of Engulf.

If the creature remains engulfed for the full rolled duration, [CENSORED] expels what remains of it into the nearest unoccupied space at the end of the creature’s final affected turn. The expelled creature falls Prone.

If [CENSORED] dies, an engulfed creature is immediately expelled into the nearest unoccupied space and falls Prone.

Severed from Beyond

When a creature fails the initial saving throw against Engulf, its connection to every source of power beyond equipment and sheer physical prowess is severed.

The creature cannot cast spells or use features whose power is magical, divine, psionic, pact-based, or granted by a deity, patron, extraplanar entity, or similar supernatural source.

Any ongoing effect requiring the creature to maintain such a connection immediately ends.

The creature retains its magic items and features derived entirely from its physical anatomy, training, or mundane equipment.

This suppression continues after the creature escapes or is expelled. It ends when Initiative ends or [CENSORED] dies, whichever occurs first.

Vomit (3/Combat)

[CENSORED] extends a [CENSORED] organ and expels a mass of thick, viscous, and violently acidic [CENSORED] throughout a 25-foot cone.

Each creature in the cone must make a DC 24 Dexterity saving throw.

On a failed save, a creature takes 38 (4d12 + 12) Acid damage. On a successful save, it takes half as much damage.

Legendary Actions

[CENSORED] can take 3 Legendary Actions, using the option below. It can take only one Legendary Action at a time and only at the end of another creature’s turn.

[CENSORED] regains all expended Legendary Actions at the start of its turn.

Heinous Smash (Costs 3 Actions)

[CENSORED] brings its entire weight down upon a point on the ground within 20 feet of itself.

Each creature in a 15-foot-radius, 30-foot-high cylinder centered on that point must make a DC 24 Dexterity saving throw.

On a failed save, a creature takes 59 (10d8 + 14) Bludgeoning damage. On a successful save, it takes half as much damage.

Every creature within 25 feet of the point, including a creature affected by the initial impact, must then make a second DC 24 Dexterity saving throw.

On a failed save, the creature is launched 30 feet directly away from the point and falls Prone.

If this forced movement causes the creature to collide with a solid surface or drag across the ground for at least 10 feet, it takes an additional 7 (2d6) Bludgeoning damage.

If the ground can be broken, the impact leaves behind a 20-foot-diameter crater. The crater is Difficult Terrain and remains indefinitely.

[CENSORED] EVERYWHERE

Optional Rule

Prerequisite: [CENSORED] has 0 Hit Points remaining.

When [CENSORED] is reduced to 0 Hit Points, the DM can have it release one final, uncontrollable manifestation instead of dying normally.

Its body expands to Gargantuan size before erupting outward in feral waves of [CENSORED] matter. These waves fill a 200-foot-radius sphere centered on [CENSORED], wrecking, assimilating, and obliterating everything they touch.

Each creature in the area must make a DC 24 Dexterity saving throw.

On a failed save, a creature takes 110 (20d10) Force damage. On a successful save, it takes half as much damage.

Unattended objects and structures in the area automatically fail the saving throw and take the maximum possible damage.

A creature reduced to 0 Hit Points by this damage is assimilated into the expanding [CENSORED]. Its body and all nonmagical objects it is wearing or carrying are destroyed.

Such a creature can be restored to life only by true resurrection, wish, or another effect capable of creating an entirely new body.

The affected area becomes coated in [CENSORED] residue for 1 hour, following the rules of the [CENSORED] Residue trait.

After the eruption is resolved, [CENSORED] collapses and dies.

[CENSORED] Traits

[CENSORED] is an unknowable aberration that shatters sanity on sight, leaves mind-corrupting residue, devours supernatural power, and destroys bodies beyond ordinary resurrection.
