Chaos Polymorph

A creature hit by this spell will every minute be turned into a completely random object, creature or energy with approximately the same size as the hit creature. For example, a medium size creature will only turn into medium, large or small sized creatures, and will only turn into objects which aren’t more than two times as large or less than twice as small as the creature.

If the affected creature were to enter limbo or the Abyss the effect will be eternal, and they will turn into absolutely anything every 30 seconds, and also the creature would be unable to enter any plane but the Abyss, Limbo or the ethereal plane without instantly be annihilated.
