You surgically combine two or more undead creatures into one. Make a Wisdom ( Medicine ), with a DC specified later. If the check fails by 6 or more, you fail to create a creature and the components are wasted. If you fail by 1 to 5, the creature has a vulnerability chosen by the DM. If you succeed by 1 to 5, you create the creature at standard values, specified later. If you succeed by 6 to 10, the creature has an extra ability chosen by the DM. As standard values, the resulting creature has hit points and armor class equal to the average of its components, plus your spellcasting ability modifier times your level (for hit points) or just your modifier (for AC). It also has up to four abilities and up to three actions, plus (if you wish) a multiattack action. These abilities and actions are taken from the component creatures. The creature also has senses, languages, resistances, vulnerabilities and immunities taken from its components. It can have up to three of each. The creature's size is determined by the size of its components, measured on a scale of 1-10, 1-2 being Tiny, 3-4 Small, 5 Medium, 6-7 Large, 8-9 Huge and 10 Gargantuan. A Tiny creature adds 0.5 to this equation, a Small creature 1, a Medium creature 1.5, a Large creature 3, a Huge creature 5 and a Gargantuan creature 8. Each size category adds 5 to the DC of creating the creature, starting at 10 for a Tiny creature.
At Higher Levels. When you cast this spell with a spell slot of a level higher than 5th, the DC for creating the creature decreases by 5, with a minimum DC of 5.
