Blair's Revivify

You touch a pile of biomass and transfer the biomass components into a creature that has died within the last minute. Roll a DC20 medicine check. On a success, the creature returns to life with 1 hit point and all missing limbs are restored. On a fail, the creature's corpse becomes a revenant which is hellbent on killing the character who cast Blair's Revivify on it.

The quantity of the biomass must be equal to 3 times the weight of the revivify target. The biomass must be from creatures of the same type as the revivify target.
