You transform into an ancient great wolf that is the size of a horse, your stats remain unchanged except for health points at 30 (40 on the 4th level of the spell. 50 on 5) Agility which is increased by 4 (5 out of 4 spell level, 6 out of 5). and your intelligence modifier is reduced by 6. you can make 3 attacks. when you attack your claws and fangs, you throw an Unarmed Strike and the damage counts 2d6 + 2 (+3 on the 4th level of the spell. +4 on 5). after spell ends / interrupts, you trow intelligence save if you fail below 13 you are unconscious for 10 minutes. if you roll below a 9 you go mad and attack everything nearby or whatever the DM chose for 20 minutes. then the only way to stop you is with a calming spell.

The transformation lasts for the duration for each target, or until the target drops to 0 hit points or dies. The target assumes the hit points of its new form, and when it reverts to its normal form, it returns to the number of hit points it had before it transformed. If it reverts as a result of dropping to 0 hit points, any excess damage carries over to its normal form. As long as the excess damage doesn't reduce the creature's normal form to 0 hit points, it isn't knocked unconscious. The creature is limited in the actions it can perform by the nature of its new form, and it can't speak or cast spells.

The target's gear melds into the new form. The target can't activate, wield, or otherwise benefit from any of its equipment.
