You unleash the power of the Fell Dragon Queen, Tiamat. You can unleash this power in two ways, changing the property of this spell depending on which version you cast:
Avatar: You allow Tiamat's power to consume you, transforming you into an Avatar of Tiamat. Your game statistics, including mental Ability Scores, are replaced by the statistics of the new form. You retain your alignment and personality. You assumes the hit points of its new form, and when you reverts to your normal form, you return to the number of Hit Points you had before you transformed. If you reverts as a result of dropping to 0 Hit Points, any excess damage carries over to your normal form. As long as the excess damage doesn't reduce your normal form to 0 Hit Points, you aren't knocked Unconscious.
You are limited in the Actions you can perform to those presented in the Avatar's stat block, as well as those that are inherent to all creatures (such as dashing or grappling). You can't cast Spells but you can use Blood Magic that you have learnt.
Your gear melds into the new form. You can't activate, use, wield, or otherwise benefit from any of your Equipment.
Shadow: You summon a manifestation of Tiamat's will. You conjure Tiamat's Shadow at an unoccupied space you can see within 60 ft. This form uses the Tiamat's Shadow stat block. The creature is an ally to you and your companions. In combat, the creature shares your initiative count, but it takes its turn immediately after yours. It obeys your verbal commands (no action required by you). If you don’t issue any, it takes the Dodge action and uses its move to avoid danger. The creature disappears when it drops to 0 hit points or when the spell ends.
When you cast the spell in this way, the spell's school changes to conjuration.
