You weave foul magics such that you make an otherwise temporary spell into a permanent curse. The Vistani wizard Alisandro Tselikov invented this loathsome spell some hundreds of years ago, to exact revenge for what he considered a great wrong.
Such is this curse's power that you can target any creature you wish, no matter how far from you, provided you are both on the same plane of existence.
As part of casting the curse, you also cast another spell you wish to be the curse. This can be virtually any spell, from chill touch to Otto's irresistable dance. The curse spell can't be used beneficially, and it can't be used to badly harm the cursed creature; in the above examples, the target wouldn't be able to use chill touch to damage themselves (perhaps by scratching their nose), but they could never touch a loved one again; and they wouldn't die from exhaustion from dancing all the time (though watching them sleep would be entertaining in itself).
The only way to remove this curse is via a wish spell or the intercession of a deity.
The ritual required to cast this spell is an act of despicable evil.
