Magically reattach the severed limb of a creature by holding the limb to the stump for the full casting time of the spell. The limb must be from that same creature unless this spell is cast at a higher level (see At Higher Levels) and must have been severed for less than three days unless it has been somehow preserved (see below). The target creature does not need to be alive at the time of casting. "Limb" in this context means any body part: internal organs, for example, can be reattached if the creature is cut open and the organ is placed inside the creature in approximately the right place. If the body part being reattached is very small (e.g. a toe, fingernail, a small patch of scalp, an eyeball) then the casting time of the spell is reduced to 1 Action.
If no effort is made to preserve the limb or the body of a dead creature it is being reattached to, the flesh becomes too decayed to be reattached after three days. This can be extended to one week if the the limb is kept below freezing temperature, one month if the limb is pickled, and potentially an infinite duration of time if the limb is somehow preserved by magical means.
Assuming either the limb or the rest of the creature contains a brain (such as if a creature's head is being reattached), the consciousness of the resulting creature belongs to the brain.
At Higher Levels. If cast at 4th level or higher, the limb being reattached can be from any creature of the same race/species (e.g. A human limb being attached to a different human, an orc limb being attached to a different orc, a dog limb being attached to a different dog). If cast at 5th level or higher, the limb being reattached can be from any creature with the same basic arrangement of limbs - for example, a halfling's arm could be attached to the stump of a elf, but that arm could not be attached to a cow or an bird. If cast at 6th level or higher, the limb being reattached can be from any creature, and does not have to be the same kind of limb (for example, the arm of an octopus could replace the tail of a cat).
* - (One cubic inch of flesh or bone from an animal or monster capable of regrowing lost limbs naturally, such as a skink, axolotl or hydra (consumed upon casting the spell))
