You touch a dead creature that has been dead for no more than a decade, that didn't die of old age, for the purpose of transplanting a specific undead soul from another vessel. The spell forms a new adult body from the corpse for the soul to enter. If the body's original soul is free and willing, it may also attempt to return to life. Part of the casting time is used to entreat the original soul not to return.
Casting this spell to restore life to a creature that has been dead for one year or longer taxes you greatly. Until you finish a long rest, you can't cast spells again, and you have disadvantage on all attack rolls, ability checks, and saving throws. Odds of failure vary by level of preparation and luck.
The magic will likely cause the creature’s race to change. The GM rolls a d100 and consults the following table to determine what form the creature takes when restored to life, or the GM chooses a form.
