Apon speaking the phrase "Memento mortis, quia venit" the user deforms and morphs into a homunculus with a portion of the power of Frosnaught, God of the Major Arcana.
While in this form, the user can perform two attack actions per turn, choosing from the following:
Endless Sight (Allows user to harmlessly view any of Maerlyn’s Rainbow, and reap their benefits. The effects of the 13 Glasses are to be determined by the DM because it would be too cluttered if I tried to fit all of them into this one paragraph.)
Hellish Maw (Summons Cerberus, defer to the Cerberus block for stats)
Nuclear Fusion (Deals 16d12 Fire, 6d12/T Burning , 6d12/T Poison to targets within a 300 ft radius. Fallout remains for 2d20 minutes)
Absolute Zero (Freezes the area in a 100 ft radius of the caster, DC19 Dex save. Creatures take 24d12 cryonic damage upon a failed save and are frozen in place for the duration of the spell. Spell lasts for 1d20 mins)
Dirt Rod (Moves a massive sphere of earth, with a radius of 150 ft from the epicentre, out of the ground and slams it into the target, 35d12, DC22 Dex save. Target takes full damage on failed saves and half on success.)
Necronomicon (Allows the user to raise any dead creature from CR1-CR30 to fight for them. Defer to the creatures monster block for stats. The creature turns into an amount of dust equal to its mass upon re-death or when the user reverts to their original form.)
Murasama (2d20 Slashing or Piercing, 18d12/T Poison for 1d12 turns. User has disadvantage on all attack rolls wade with this weapon.)
Barometer (Use the command word to summon a massive, discordant storm which randomly strikes creatures with lightning, hail or summons tornados. 6d100 Electric, 16d12 Bludgeoning or 10d12 true damage respectively. DC35, DC16 or DC18 Dex saving throws respectively. Target takes full damage on failed saves and half on success.)
I'm aware that this spell is obscenely overpowered. I originally made it for use against a party of 5 level 40 players. Yes, I tortured myself for setting up an extra 20 levels of stats, feats, spells, etc. for a single campaign.
