When you activate this you will have to roll a saving throw. If you fail the saving throw you will go on a rampage. you have to kill a minimum of 5 people to escape this rampage. But if you succeed on the saving throw from there after you can use it for as long as you like. What this does is it puts you in a state where if you kill someone in this state you will be able to absorb one of their stats that you can then keep.
