Turn into X. The first casting of this spell in a day does not consume a spell slot. X is equal to a list provided by your dm.

If cursed all forms are cursed

Same with exhaustion

Become self fails if grappled or if a creature is holding you. Or if you become frightened. 

If any of the forms take damage, the total Hp is represented by the lowest number between all forms. 

In addition all spell slots are shared, but only available to be used by an appropriate character.
