Damage/Effect
You place a curse on a creature that you can see within range that is no bigger than Large, branding it as the "Protagonist". The area around the Protagonist is enveloped in a darkness that fills a 15-foot-radius sphere. A glowing orb also hovers above the Protagonist, shedding bright light onto a 5ft square, following it. Creatures in the area are unable to leave the boundaries of the spell and all interaction is blocked between the inside and outside of the area. You the caster, are branded as the "Antagonist" and other creatures who are not branded are treated as the "Audience". Audience members can interact with each other but are unable to interact with the Protagonist or Antagonist. The Protagonist is subsequently subjected to different effects each round referred to as 'Acts', starting with Act 1 when you cast the spell. Should either the Protagonist or Antagonist die, the spell ends immediately. Creatures immune to disease are unaffected by this spell.

Act 1. A Sudden Illness - A disembodied voice called the Narrator speaks out "Hope is like the sun, which as we journey toward it, casts the shadow of fate behind us. Pestilence is the fate hiding in your shadow" As the Protagonist's skin begins to malt and welt. It must make a constitution saving throw or have an illness sweep their body from within. The target has its speed reduced by half and takes 10d6 necrotic damage with a fail, or half as much damage with a successful one.

Act 2. False Hope - The Narrator continues "The hand of the surgeon is hard but healing. They are beacons of light for those who are stricken." As the Protagonist is teleported and magically restrained onto a stone surgical table. A phantasmal illusion bearing the appearance of the Antagonist's form of dread stands over the Protagonist, cutting it open with great precision and callousness. The Protagonist must make a Wisdom saving throw. It becomes frightened of the Antagonist and takes 10d6 psychic damage with a fail. On a successful save, a creature takes half as much damage and isn't frightened.

Act 3. The Grave Finale -The Narrator says "For all, it is inevitable. Although it is arrives sooner for some. The only correlation between life and death, is that life is the biggest lie we believed, while death; is the largest truth." As the stone surgical table warps into a tombstone around the Protagonist and is set ablaze. It must make a strength saving throw or take 10d6 bludgeoning damage and 10d6 fire damage on a failed save, or half as much damage on a successful one. If the creature dies during Act 3, it's tombstone is buried 100ft below the surface. Opening the tombstone will reveal the ash of its burnt remains.
