The caster takes a living humanoid as a sacrifice to the Blood God. The creature is cut open and the blood is drawn out of them by dark magic causing them to die from exsanguination. The blood swirls in the air in front of the caster creating a portal to a desired, previously visited location. The portal exists until the caster passes through or until 30 minutes have passed, at which point the blood is violently splashed around the area of the portal in tribute to Khorne.
This action is abhorrent to any creature not Evil and can even make any who are Good who see this attack on sight. If the caster is not Evil they must make a Constitution saving throw of 12 or higher each time they cast a spell after Blood Portal, otherwise the damned screams of the sacrifice fill their thoughts giving disadvantage on Spell rolls. This effect lasts 1 in game day or until a Constitution saving throw of 20 is rolled.
