Blink dog blade

You summon the Blink Dog Blade. It is a unique dagger crafted of obsidian and mounted on the jawbone of a blink dog. When this dagger hits a target or object within ranged attack range, the thrower may choose to teleport as a bonus action to a space within 5 feet of the hit target. This action does not break stealth if no damage is applied. Alternatively, the thrower may choose to not blink and have the dagger return to its sheath instead. The dagger does 1d6 piercing damage.

At higher levels: You add 1d6 for each level above 1st.
