You alter your own or a willing creature's physical form, causing them to take on a new shape. The target's body transforms into a new form of your choosing. The new form can be any creature you are familiar with, or a custom form of your own design. If you attempt to cast this on an unwilling creature, they must make a Constitution saving throw or have their form altered.
The new form must be of the same size category as the target's original form, and must have a similar skeletal structure. The target's equipment and possessions meld into the new form, and the target retains its hit points and hit point maximum. However, the target's physical abilities and statistics are replaced by those of the new form, as follows:
- The target's Strength, Dexterity, and Constitution scores become those of the new form.
- The target's speed and any special movement modes become those of the new form.
- The target gains the natural attacks and special abilities of the new form, as appropriate.
The target retains its personality, memories, and mental ability scores. If the target is in an environment that is hostile to its new form (such as a fish on land), it suffers appropriate penalties.
At the end of the spell's duration, the target reverts to its original form, regaining its original physical abilities and statistics, as well as its equipment and possessions. If the target drops to 0 hit points before the spell ends, it reverts to its original form and remains unconscious for the duration of the spell.
