This highly dangerous and experimental spell allows the caster to swap the brains of two creatures. The caster must touch both creatures throughout the casting of the spell. Upon completion, the brains of the two creatures are magically exchanged. Each creature then wakes up in the other’s body.
If a target is unwilling, it can make a Wisdom saving throw to resist the effects of this spell. The DC for this saving throw is equal to your spell save DC. On a successful save, the spell has no effect on that creature.
The spell fails if either creature’s body is unable to support the incoming brain (for example, a size mismatch or a creature without a brain). The spell also fails if either creature dies during the casting.
After the spell is cast, each creature retains its alignment, Intelligence, Wisdom, Charisma, level, class, base attack bonus, base save bonuses, languages, and mental abilities. These aspects are preserved when the brains are swapped between bodies. So even though the bodies are switched, these mental attributes remain with the original mind in the new body.
Meanwhile, each body retains its original physical attributes, including Strength, Dexterity, Constitution, hit points, natural abilities, and automatic abilities. These physical attributes are not affected by the swapping of brains and remain with their original bodies.
Upon completion of the spell, each creature must make a DC 20 Intelligence saving throw or take 2d8 psychic damage due to the mental strain of adjusting to a new body.
The effects of this spell are permanent and only reversible by the use of the spell Brain Exchange or Wish.
