This spell transforms a creature that you can see within range into a new form. An unwilling creature must make a Wisdom saving throw to avoid the effect. The spell has no effect on a shapechanger or a creature with 0 hit points.
The transformation lasts for the duration, or until the target drops to 0 hit points or dies. The new form can be any beast whose challenge rating is equal to or less than the target's (or the target's level, if it doesn't have a challenge rating). The target's game statistics, including mental ability scores, are replaced by the statistics of the chosen beast. It retains its alignment and personality.
The target assumes the hit points of its new form. When it reverts to its normal form, the creature returns to the number of hit points it had before it transformed. If it reverts as a result of dropping to 0 hit points, any excess damage carries over to its normal form. As long as the excess damage doesn't reduce the creature's normal form to 0 hit points, it isn't knocked unconscious.
The creature is limited in the actions it can perform by the nature of its new form, and it can't speak, cast spells, or take any other action that requires hands or speech.
The target's gear melds into the new form. The creature can't activate, use, wield, or otherwise benefit from any of its equipment.

I always thought you could polymorph into anything, and it was only worse than true polymorph because it's only creature to creature. Maybe you should be able to polymorph into anything that's not a construct or undead.

Can you cast polymorph on your familiar? Or can you cast twin Spell on Polymorph to make 2 giant apes.
Can you choose what they turn into? or is it random because from this it just seems like the dm picks one
Hey, new guy here. I’ve been playing D&D for only about a year, but I love it so much.
Please help me figure one thing: if I (a wizard) were to use Polymorf on myself to change into a Giant Ape, would I still need to check for Concentration every time I (in the form of the Giant Ape) get hit?
Because once one creature is Polymorfed, it uses the stats of the Beast: Wis, Int, Con etc.
It cannot use ANY of the old stats. It cannot speak or (in most cases) understand any languages.
The Beast only knows who its allies are, so from this I presume it doesn’t have any of the “old” thoughts (i.e. the wizard’s thoughts).
So how come I still need to make a Concentration check, if I (the Giant Ape) don’t even know what a spell is? I’m a newborn, I’ve been here only for a few seconds.

Bruh Polymorph + Power Word Kill can kill everything… maybe you can choose a divination wizard so the target can’t roll Saving Though on Wisdom at all… this is POWER!!! I can kill terrasque btw…
So what happens if you use this turn an enemy into a fish? Will the penalties for being a fish out of water go away when they revert?
if there was a unkill able monster if poly morphed into a rabbit if you behead said rabit monster still dies from rules defined in vorpal sword stat sheet
if you Polymorph into a creature that has legendary action, can you use them? because i know that you can't with wild shape. (dose this also count with true polymorph)
I think as long as it has the proper anatomy it can do the things. For example and ape performing flurry of blows or stunning strike seems completely legal to me.
fish can die very soon when out of the water. I prefer beasts which will not die during the spell duration (1 hr) ;)
conditions (when they revert) depend on the environment where they will revert (you can throw polymorphed enemy somewhere - into water, into abyss, into trap,... )
the tunnel expodes? or spell fails.
