You attempt to swap bodies with a humanoid creature you touch. If the target is unwilling, it must make a Charisma saving throw. On a successful save, the spell fails.

Your mind and soul transfers into the body of the target. You retain your alignment, personality, and Intelligence, Wisdom, and Charisma scores. You also retain all of your skill and saving throw proficiencies, as well as your proficiency bonus. You retain all abilities from your class and feats, and can use them if this body is physically capable of doing so.

You retain benefits from your race that are attributed to your culture or mind, such as a dwarf’s stonecunning or dwarven weapon training, but not benefits associated with your physical body, such as darkvision. You gain the physical benefits from the race of the target, but not the cultural or mental benefits.

You assume the target’s hit points, hit dice, and Strength, Dexterity, and Constitution ability scores. You look and sound exactly like the target, and have advantage on Charisma (Deception) checks to pass yourself off as them.

You also have all of the target’s equipment. You automatically attune to any magic items the target was attuned to, and unattune from any magic items you were attuned to. If the target was attuned to any cursed magic items, sentient magic items, artifacts, or other items that had a special bond with the target beyond ordinary attunement, those items will not attune to you, and will remain attuned to the target’s soul. Otherwise, if you don’t meet the requirements to attune to one of the target’s items, it will not attune to you, but it will unattune from the target.

Any magical effects such as spells or curses on you transfer to this new body. You take on any physical afflictions or conditions affecting the target’s body, such as poisons or diseases.

The target’s mind and soul transfer into your body, following the same rules.

If one of the bodies involved in the swap dies, the mind and soul currently inhabiting it dies. When the spell ends, the body that the remaining mind is in will transmute to be identical to its original body in appearance and capabilities, except for its equipment. The corpse of the dead body will also transmute to look like the body of the soul which died with it.

This spell can be ended early if both creatures mutually agree to end it with a handshake.
