You touch a dead humanoid or a piece of a dead humanoid. Provided that the creature has been dead no longer than 30 days, the spell forms a new body for it and then calls the soul to enter that body. If the target's soul isn't free, the spell fails. If the soul is unwilling, it must make a WIS saving throw equal to your spell save DC. If the soul fails the save, the spell takes effect. If it succeeds on the save, the spell fails.

The magic fashions a new body, that of a beast, for the creature to inhabit. The GM rolls a d100 and consults the following table to determine what form the creature takes when restored to life, or the GM chooses a form.

The reincarnated creature recalls its former life and experiences. Its game statistics, including mental ability scores, are replaced by the statistics of the new form. It retains its alignment and personality.

The creature is limited in the actions it can perform by the nature of its new form, and it can’t speak, cast spells, or take any other action that requires hands or speech, unless its new form is capable of such actions.

The target’s gear melds into the new form. The creature can’t activate, use, wield, or otherwise benefit from any of this melded equipment
